ST Case 15

Poor signal quality

Summary

A 38 year old G8 P7. Gestation week  40+3 admitted to hospital in spontaneous labour contracting since early morning.  Normal pregnancy.  VE: Cx fully effaced, dilated 6 cm, vx -1.

  • 16:40 Admission CTG test is normal.
  • 18:18 AROM, thin meconium stained fluid. FSE applied and ST Analysis initiated. VE: Cx fully dilated, vx+2.
  • 18:30 Active pushing started.
  • 18:45 Spontaneous vaginal delivery.

Comment

This is a case with poor signal quality throughout the tracing with an odd ECG waveform. The T/QRS baseline is never determined and the ST information can therefore not be used for assessing the fetal state. The TOCO transducer is not applied properly, labour progresses very quickly and the patient delivers after only a few pushes.

Reminder

  • Gaps in the T/QRS ratio >4 minutes may result in missed ST Events.
  • Check for Event Log message ” Baseline T/QRS determined”
