ST Case 11

Maternal pyrexia. Stan GL followed.

Summary

26 year old  G1 P0. Gest.week 41+3.  Normal pregnancy.

  • 10:10 Admitted to hospital contracting and SROM since 32 hours. VE: Cx fully effaced, dilated 6 cm, vx -2. Clear liquor draining.
  • 11:11 Epidural sited on maternal request.
  • 12:30 Oxytocin infusion started. VE: Cx fully effaced, dilated 7 cm, vx -1.
  • 14:30 VE: Anterior lip, vx at 0 station. Maternal temp 37,8. One dose of antibiotics given i.v.
  • 15:14 One Biphasic ST in the Event Log.
  • 15:31 Second Biphasic in the Event Log.  Intervention required. Physician assessment, decision made for Caesarean section.
  • 15:54 Emergency Caesarean section. Babygirl born. AS 9,10,10. Birthweight 4 410 g.

Comment

There is One Biphasic ST Event  in the Event Log at 15:14, the CTG trace is Abnormal. At 15:31 the Second Biphasic ST event appears in the Event Log and according to Stan GL: Intervention is required,  if no improvement of fetal state,  delivery. In this case immediate decision without delay.

IMPORTANT to remember: In the presence of  maternal pyrexia, even intermediary CTG pattern may be regarded as significant in combination with ST Event.
