ST Case 13

Fetal head pressure.

Summary

37 year old G3 P2. Gest. week 40+3. Unremarkable pregnancy.  

  • 01:20 Admitted to the hospital contracting for a few hours. Patient requesting pain relief as soon as possible.  SVE: Cx 6 cm dilated, 100 % effaced, vx -1.  Bulging membranes.
  • 02:00 SROM with clear fluid noted.
  • 02:07 FSE applied. ST Analysis initiated.
  • 02:14 Poor signal quality noted.
  • 02:27 FSE reapplied, improved signal quality.
  • 02:49 Biphasic ST Event.
  • 03:30 Active pushing started.
  • 03:34 Episodic ST Event.
  • 03:37 Spontaneous vaginal delivery of baby boy, birth weight 3 080 g. Apgar Score 10/10/10.

Comment

Remember it takes two Biphasic ST Events in the Yellow Zone to change the management recommendation from that of “expectant management, closer observation” to that of “direct physician assessment, intrauterine resuscitation as appropriate, if no improvement in fetal condition, expeditious delivery”.

The delivery occurred minutes after an Episodic ST Event was displayed during a Yellow Zone tracing.
