Intervention

Summary

38 year old G1 P0. Gest. week 38+3.  Admitted to the hospital in spontaneous labor.  Unremarkable pregnancy.

  • 15:05 Admitted to the hospital in spontaneous labor. SVE: Cx 3 cm dilated, 100% effaced, vx -2.  SROM occurred with vaginal examination, clear fluid noted. Epidural placed.
  • 15:44 FSE applied. ST Analysis initiated. Oxytocin infusion started.
  • 18:00 SVE: Cx 8 cm dilated, 100% effaced, vx  0 station.
  • 19:48 Labor continues.  Baseline ST Event occurred at 19:48 and Oxytocin infusion was decreased due to tachysystole between 19:20-19:30.
  • 20:00 FHR pattern classified as a Green Zone tracing.
  • 21:10 Active pushing started.
  • 23:20 SVD 3560 g. Apgar Score 9/9/10.

Comment

During a Yellow Zone tracing, in the first stage of labor, with a Baseline ST Event the STAN guideline recommends intrauterine resuscitation as appropriate.  In this case tachysystole was addressed and resolved.  The fetal condition improved, the FHR pattern was classified as a Green Zone tracing and labor continued.
